All caregivers must provide a copy of the information listed below:
- Completed Application
- Level 2 background screening. If you do not have a current screening you must obtain one at your expense.
- Driver Licenses, Social Security Card, CPR Certification
- Proof of Car Insurance
-HIV/AIDS Certificate of Completion
-CNAs- current licenses
-HHAs- Proof of 40 hours of training or a Certificate
